MEDIA CONSENT/RELEASE

I hereby authorize WHAT-A-CANINE, LLC to use photos and/or videos of myself, my dog, and/or my family members and/or information related to my experiences with WHAT-A-CANINE, LLC. I am over the age of 18 and/or am the parent/legal guardian of a minor in the photos/videos.

I understand this information may be used in publications, including electronic publications, audiovisual publications, promotional literature, advertising, social media, media, and/or other similar ways. WHAT-A-CANINE, LLC will disclose to me or my legal representative, where appropriate, the specific information and/or photo/video to be used prior to release on social media or any other publication.

My consent is freely given without expecting payment. I release WHAT-A-CANINE, LLC and their respective employees and agents from any and all liability which may arise from the use of such news media stories, social media posts, promotional materials, written articles, videotape and/or photographs.

I understand that I can revoke this release at any time by requesting in writing and that the use of any of my photos or videos or other information authorized by this release will immediately cease.
